Chronic Refractory Cough Diagnosis

The diagnosis of CRC is either done by primary or secondary assessment. The initial assessment for protracted or chronic cough is intended to characterize the condition based on history, to elicit any alarm symptoms or findings that may indicate a serious underlying disease, and to identify whether there is a specific disease present that is associated with chronic cough. A second stage investigation is considered to further characterize the condition of the disease. These include nasendoscopy and 24 hour pH monitoring. Chest radiograph, spirometric and peak expiratory flow measurements, bronchoprovocation testing, gastrointestinal investigations, high resolution computed tomography (CT) of the thorax and bronchoscopy are some of the common tests performed to examine chronic cough.

There are currently no approved therapies for CRC. However, various off-label and over-the-counter medications are being used to treat the condition. These include neuromodulators (like opioids, baclofen, gabapentin, pregablin, and amitriptyline), proton pump inhibitors, inhaled corticosteroids (ICs) and others.

CRC is caused due to the recurrent occurrence of chronic cough. However, the signs and symptoms of chronic cough are similar to that of CRC. Persistent or chronic cough is one of the first and important symptoms and may signify respiratory involvement. Other pulmonary symptoms and signs include wheezing, stridor, dyspnea, hoarseness or aphonia, and tenderness over the laryngotracheal cartilage. Key symptoms include a dry irritating cough localized around the laryngeal region. However, the symptoms are not restricted to cough and can include globus, dyspnea, and dysphonia.

In another survey performed in about 1120 people with chronic cough, more than 70% of the subjects benefited from more than three consultations in connection with their chronic cough; however, a diagnosis was given in about only 53% of the cases while 2.7–46% of chronic cough was represented as refractory in nature. Also, there were several risk factors associated due to chronic cough where smoking contributed to be the major one.
  • The general population, chronic cough had a prevalence of 9–33% in the USA and Europe. The most important risk factor for chronic cough was cigarette smoking. Prevalence of chronic cough was three times higher in chronic smokers as in never- or ex-smokers. It was also highlighted in some studies which, authors had referred and reported that approximately up to 80% of patients experienced chronic cough associated due to idiopathic pulmonary fibrosis, however, lower numbers were also reported.
